FMEA / FMECA / FTA & Annual Trials

dynpos Ltd undertakes Failure Modes & Effects Analysis (FMEA), Failure Mode & Effects Criticality Analysis (FMECA) and Fault Tree Analysis (FTA) projects from desktop studies through to the FMEA Proving Trials.

Failure Modes & Effects Criticality Analysis (FMEA/FMECA)

The FMEA of a DP vessel is based on a single failure concept under which each system??s subsystems and parts are assumed to fail by one probable cause at a time. It must in effect make sure that the stated worse case failure for the vessel cannot be exceeded by any single failure.

Most analyses of this type also include some method to assess the risk associated with the issues identified during the analysis and to prioritise corrective actions. The method utilised by dynpos Ltd are:

  • Criticality Analysis (FMEA with Criticality Analysis = FMECA)
  • Mode Criticality = Item Unreliability x Mode Ratio of Unreliability x Probability of Loss

Fault Tree Analysis (FTA)

Fault Tree Analysis (FTA) is a technique that provides a systematic description of the combinations of possible occurrences in a system, which can result in an undesirable outcome. This method can combine hardware failures and human failures.

Where human life is at the forefront of the operation such as divers on a DP Dive Support Vessel this is an essential tool in further reducing risk and ensuring their safety.

To include human error, a detailed Task Analysis is first required, breaking down the detail of the actions to be done, taking account of conditions, speed of operation and the correct sequencing of individual actions.

Possible deviations can then be identified. After allowing for shaping factors, which influence individual performance, (such as skill, stress etc.), and recovery factors, (most human errors are recoverable), the contribution of human error can be estimated, by using data on human error rates.

Annual/FMEA Proving Trials

dynpos Ltd can witness annual trials based on the vessel’s FMEA.
